Primary care extended access survey (22 January – 13 February 2018
Have your say on access to NHS GP services outside of the current opening hours by completing a short survey.
This survey will give you the opportunity to say when you would like to be able to book extended access GP appointments, where you’d be willing to travel to these services and your views on alternatives to face to face consultations. The survey will take no longer than 5 minutes to complete and is anonymous.
Your views will help inform discussions with potential providers going forward and shape the service we commission locally.

Please note the online survey is being hosted on East Sussex County Council’s consultation hub and they will be processing responses and analysing data on behalf of the CCG’s. If you chose to take part, your answers will be treated in confidence.
Paper copies are available from your GP surgery and once completed you can drop it back to your surgery or post it for free to; NHS GP Survey, FREEPOST SEA2472, BN8 2ZZ. Please ensure all responses are received by Tuesday 13 February 2018.
